I confirm the health information I provide is accurate and complete. I will disclose any medical conditions, injuries, medications, allergies, skin sensitivities, recent surgery or pregnancy before treatment. I understand that massage and bodywork at FLOW is not a medical diagnosis, treatment or cure, and is not a substitute for professional medical advice, diagnosis or care from a GP or other qualified practitioner.

I understand that after a massage it is normal to experience temporary soreness, tenderness, fatigue, light headedness or minor bruising, and that these effects are not grounds for a refund or claim. I agree to follow the aftercare advice given and to contact a medical professional if I have any concerns.

I consent to massage and bodywork treatment at FLOW Wellness Sanctuary. I understand that the therapist may adapt or stop treatment at any time for my safety or comfort, and that I may ask for the session to end at any time.

Privacy notice — I understand FLOW stores my contact and health details securely, uses them only to deliver and manage my treatment, never sells or shares them with third parties for marketing, and retains records as required by insurance. I may request access or deletion at contact@flowmobilewellness.co.uk.

I accept that a 25% deposit is required to confirm my booking and is non-refundable for cancellations within 24 hours of the appointment.
